Download the Excel File or fill out the Form on line.  The employee information requested on the Census Information Form is used to calculate the contribution amounts for each employee of the company.  Accurate calculations depend on the receipt of all the requested information for each employee.

The Employee Data for the Period Beginning and Ending is a 12-month period normally beginning on the first day of the Fiscal Year and ending on the last day of the Fiscal Year.


The Dates of Birth and Dates of Hire are used to determine employee eligibility, participation dates for calculating the plan and age discrimination testing.


Total Compensation includes W-2 wages, Partnership K-1 income or the Sole Proprietor's net Schedule C income.


The Paid Hours should include all hours an employee works or is considered to have worked.Please download, complete and return via E-Mail or facsimile both the Sponsor Information and Census Information Forms.  A Proposal, including a fee quote, will be E-Mailed to the sender unless otherwise requested.  After the Proposal has been reviewed, you may E-Mail LDSCO to schedule a meeting to discuss the Proposal.
